May someone help clarify (with as much detail as possible) what is "the bid form."   Is there an AIA template?

  AIA document A701 Instructions to Bidders which you reference above notes Article 4 Bidding Procedures which explains what the bid form needs to include. The bid form is typically provided by the owner or GC within a "bid package" and acts as a proposal in response to the Invitation to Bid. Bid forms typically include Contractors total project cost, breakdown of the contractor's project scope including labor, materials, equipment; estimated project schedule (start & completion dates), payment terms (how the contractor will be paid); signature binding the contractor and GC.
