Who can order a contractor to pick a new subcontractor?

I’ve seen conflicting answers on this question. If the architect wants the contractor to choose a new subcontactor, for whatever reason, does the owner have to approve the change or is the architects request enough?

  Here is the official A201, 5.2.3 language about this, the objection has to be reasonable, and the contractor may ask for a change order:

  I have a follow-up question. 
  What if the Owner or Architect wants the Subcontractor fired from the job after a 14-day period of notice for reasonable objection expired and the Subcontractor already started Work. I couldn't find any provision for such action in A201. Can the Owner or Architect request to change Subcontractor after the window for objection expired and the Subcontractor started the work?
